To Schedule an initial appointment or consultation:
- Click on the 'New Patient Paperwork' button above and fill out the forms.
- You will be prompted to open up an online portal account through Practice Q, our online practice management system.
- Once you fill out the forms online, which includes adding your credit card, then we can we confirm an appointment time. We may give you some available times in advance, but nothing will be confirmed until these steps are completed. You will also receive an email of the appointment confirmation.
